Human Subjects Form is on your PORTAL (blackboard, email, etc.)Sign into the my.hbu.edu (Portal). View on the top the HCU Forms. Find Academic Forms. Look for "Human Subject Protection...." ASK to make sure this is the right form. Print it out and follow directions. I think your professor must sign as must student/s applying the test. Submit to IRB for approval of study.
